Patient Billing Facts

COVID-19 testing could be reimbursed by your health plan or the government, with no out of pocket cost, when billed to your health plan or the government. Kodiak Testing will submit a claim to your active health plan on your behalf ranging from $250.00 to $300.00.

The claims process can be lengthy and confusing, and we want to make it as easy for you as possible. The information below may be helpful.

Explanation of benefits (EOB) from your insurance company: You may have already received a written summary of the claim we submitted and its status from your insurance company. This document is called an Explanation of Benefits (EOB). This is not a bill.
